Setting age verification in questions

You can add an age verification question to your survey using the following methods.

Method 1:

Use a Yes/No question and a Validation Message question

  1. On the Edit tab, click Add Question.
     
  2. From the Question Gallery, click the Yes/No (vertical) or Yes/No (horizontal) question type.
     
  3. In the Yes/No question editor, enter the age verification question, click Answer required, and click OK.

    Next you will add the Validation Message that will validate the response to the Yes/No question. Validation messages always follow the question that they are validating.
     
  4. On the Edit tab, click Add Question.
     
  5. From the Question Gallery, click the Validation Message question type.
     
  6. In the Validation Message question editor, enter the text you want to display if the respondent is not of age.
     
  7. Click Ask if.
     
  8. From the Ask if page drop-down menu, choose the Yes/No question and click OK.
     
  9. Click the No option so that this Validation Message question will be asked if the respondent answers "No" in the Yes/No question. Click OK.

  10. Click OK again to return to the Edit tab. If there is no page break between the Yes/No question and the Validation Message question, Vista's error correction will display a Fix it for me prompt. Click Fix it for me to add the page break.
     
  11. Click Test Logic to test your Validation Message question.

Method 2:

Use a Validation Message question with a formula

  1. On the Edit tab, click Add Question.
     
  2. From the Question Gallery, click Question Library.
     
  3. Choose Date Chooser or Date Chooser (use with form header) from the Question Library drop-down list, and click Add Question.

    The Date Chooser question is added to the questionnaire. 
     
    Next you will add the Validation Message that will validate the response to the Date Chooser question. Validation messages always follow the question that they are validating.
     
  4. On the Edit tab, click Add Question.
     
  5. From the Question Gallery, click the Validation Message question type.
     
  6. In the Validation Message question editor, enter the text to display if the respondent is not of age.
     
  7. Click Ask if.
     
  8. From the Ask if page drop-down menu, choose Other, and click OK.
      
  9. On the Ask If Conditional page, enter the following equation and click OK:

    now-todate([Q1])<18yr

    [Q1] refers to question 1 in the questionnaire. If the Date Chooser question that you added is the third question in the questionnaire you will replace [Q1] with [Q3].
     
  10. Click OK again to return to the Edit tab. If there is no page break between the Date Chooser question and the Validation Message question Vista's error correction will display a Fix it for me prompt. Click Fix it for me to add the page break.
     
  11. Click Test Logic to test your Validation Message question.
