Form Header Question

The Form Header question is used to start a Form Question, a vertically aligned question group, in a survey.  Here is an example of a Form Header question.

To add a Form question group to a survey, you first add a Form Header then immediately after add any number of question types with the Form layout (right column of the Gallery).

The following is a description of the fields in the Matrix Header question editor:

  • Header Text - Entry field for your header text.  You can use the formatting toolbar to add formatting to the question such as text size, text formatting (bold, italic, underline), color, etc.
  • Ask if - Click this button to set the condition under which this question will be asked, i.e. branching and conditional logic.  For more information, see the Ask if Logic page.
